Is there any workflow rules that are needed? When should the workflow rule fire, what are the criteria? What type of workflow do you need (task, email alert, field update, outbound message)?
Task
Who should the task be assigned to? What should the subject be? What should the due date be? What should the status be?
Email Alert
What should the email say? What is the email subject? Who is the email going to? Who is the email from?
Field Update
What field are you updating? What are you updating this field too?
Outbound Message
What is the endpoint URL? Who do you want to send the email as? What fields do you want to send?
Validation Rules
Are there any validation rules that need to be put in place? What fields should this validation apply too? Who should it apply too? Should it apply to all record types?
Error Message
What should the error message say?
Error Message Location
Where should the error message be displayed? At a specific field? At the top of the page?
Page Layouts
Do you need different page layouts? Should there be a different page layout for each record type?
Sections
What sections should be on the page? Which fields should be split up between the sections? Should there be one column or two columns?
